FREE SPORTS PHYSICALS – THIS THURSDAY, MAY 13TH
Free sports physicals will be held at Chase Middle School on Thurs., May 13th from 3:00-5:30pm.
All students must have a physical and other paperwork completed prior to participating in summer workouts, practice or tryouts for next school year. This includes current 5th graders who will be at CMS next school year. Please make every effort to have your child take advantage of this free physical.
Chase Middle Physical Form Packets sent home with all students on last week. Please complete the parent sections and return to school by tomorrow. Remote student parents can come by school to pick up a packet.
