Sports Physical Packet

All middle and high school students who wish to participate in school athletics in North Carolina are required to have a current sports physical on file. A sports physical (also called a pre-participation physical evaluation) ensures that a student is medically safe to take part in athletic activities. Under North Carolina State Board of Education policies and athletic eligibility rules, students may not try out, practice, or participate in any sport without a completed and up-to-date physical submitted to the school.
